If an appointment is set for a minor, age 17 or younger, a parent needs to be present at the first appointment.
Before you come on board, we do need to get some information from you. If you can please download the following forms, complete and bring them to our first meeting,we will be able to immediately start on our session.
Before your first visit, please click and print the following forms below from our forms intake page. The Parental Guardian consent form is required only if clinician requests it from you.
